If you've recently changed your name, your Telstra account will need to be updated.

You can use this form:
- If you are a main account holder and want to update your name
- If you are an authorised representative on an account and want to update your own name
You will need to provide some personal details as well as some supporting documentation for us to process your request.
- Option 1 - Send us a copy of your old ID (showing your old name) plus a copy of your new ID (showing your new name).
- Option 2 - Send us a copy of an official document that shows that your name has changed. This documentation must be issued in Australia.
