If you wish to switch part of your home phone service to Telstra simply:
Please note: Once your request has been submitted, it will take up to 21 working days for your request to be processed. During this time, calling 12711 will confirm the transfer of your long distance calls to Telstra. Telstra will use best endeavours to notify you of your completed transfer within 10 working days or you may contact us on 132 200 to confirm if the transfer is complete.
Complete this form to switch your fixed phone line rental, local long distance (national and international) and fixed to mobile calls to Telstra. Once you have submitted your details, we will activate the transfer and notify you when this has been completed.

Remember, this form must only be completed by the telephone account holder (or authorised representative) of the fixed telephone services to be switched to Telstra.
